How does Patient Initiated Follow-up (PIFU) compare to standard care follow up for people living with inflammatory arthritis?

Full Title of study: What is the clinical and cost-effectiveness of a patient-initiated follow-up (PIFU) strategy compared to traditional pre-arranged appointments for people with inflammatory arthritis (IA) treated with long-term immune-suppressing therapies? A randomised controlled trial nested within a national rollout programme with assessment of cost-effectiveness and acceptability.

NRAS’s National Patient Champion, Ailsa Bosworth is a Patient and Public Involvement (PPI) co-applicant and part of the PPI team study team on TaILOR. Given PIFU is already being rolled out, NRAS believes this is a very important study.

How will the study team do this?

Over a four-year period, the study team will monitor quality of life and how NHS services are used by all patients switched to a Patient Initiated Follow Up (PIFU) appointments as required using information recorded routinely by hospitals and GPs across the UK from the 30+ study sites. 438 patients with stable inflammatory arthritis will be recruited to a specific PIFU trial. They will be randomly allocated to receive PIFU and an appointment in 2 years or standard care (appointments every 6-12 months). The study team will measure their quality of life over the following 2 years.

The study will assess how well their disease is controlled, their use of medications and NHS health services and patients’ satisfaction with their care. The study team will also conduct detailed interviews with patients and clinicians to understand their experience of PIFU and use this information to help clinicians advise future patients about future care.

The duration of this project runs from 1 April 2024-30 April 2028.
