Collect in-person donations and Gift Aid by downloading a sponsor form for your fundraising event here.

By downloading a sponsor form here, you can easily ask friends, family, colleagues and neighbours to support you with your fundraising!

Don’t forget, if they are a UK taxpayer, they just need to provide their full name and full address, including the postcode, so we can claim 25p Gift Aid on every £1 donation they make.

Once completed, please pay in the funds and return the original sponsorship form via post to our address below.
